Enrollment

New potential members and renewing members who were not with us last year must submit the New Members Application as soon as possible.

Mail to WWHM c/o Lisa Jaqua, 4444 S. West Street #76, Wichita, KS 67217 or email them to wwhmlisa@cox.net Upon receipt of your application we will contact you with further instructions.

Also we ask that all new and renewing members attend an Orientation before they can join. Next oreintation is Dec. 15th, 2008. New members will be given all the below forms at Oreintation.

All current members should of received an email with pre-enrollment instruction and attached to it all the forms they need to fill out.

Each year all families need to sign the Policy Agreement. Policy Agreement.

Each semester all families need to fill out Registration / Medical Form and < 2009.1 Registration Worksheet .

Also if your teacher is willing to give you a discount of any kind for example on book fees due to your kids sharing one text then you must fill out the Adjustment Fee Persmission Slip and have the teacher sign it. Adjustment Fee Permission Slip

Mail hard copies to WWHM c/o Lisa Jaqua, 4444 S. West Street #76, Wichita, KS 67217 or turn it in at the orientation/registration meeting.

The Orientation /Registration meeting will go much smoother and faster if everyone mails their forms in early. All payments are made in person at registration. PLEASE DO NOT MAIL ANY MONEY.

Membership & Class Fee Guidelines

Every family will pay a membership fee per semester per family. It is $25 for 1 hour, $35 for 2 hours and $45 for 3 or more hours that their family is taking classes. The membership fee pays for use of the facility, administrative supplies and advertising costs. Classes will be paid for at the time of registration. Classes are individually priced according to the price determined by the teacher. A late fee of $5 per class will be charged to those registering after the first week of classes.

Refund Policy

WWHM reserves the right to cancel membership for any reason. ALL FEES ARE NONREFUNDABLE. Exception: A refund will be given only if the class is canceled by the teacher and/or the minimum enrollment is not met.
